Coordinated, Evidence-Based Care Plans

We begin with a comprehensive history, vitals, medication reconciliation, and baseline labs to assess risk. Your GP then sets a personalised plan with clear goals, education on self-monitoring and red-flag symptoms, and practical lifestyle steps. We coordinate allied care (dietitian, physiotherapy, mental health) and schedule follow-ups every 3–6 months, with recalls for tests, vaccinations, and annual reviews.

General questions

We manage diabetes, hypertension, high cholesterol, asthma/COPD, thyroid disorders, arthritis, cardiovascular disease, obesity-related risks, and common multi-morbidities. If a condition needs specialist input, we coordinate shared care and referrals.
Most stable conditions are reviewed every 3–6 months. We’ll see you sooner after medication changes, flare-ups, or new symptoms, and complete a comprehensive annual review with updated tests.
Bring your ID, medical aid details, medication list (or photos of labels), home readings (e.g., blood pressure or glucose logs), and recent test results or hospital letters. Devices like a glucometer or BP cuff are helpful.
We send structured referral letters, share relevant results, and agree a clear care plan. Feedback from specialists and allied clinicians is tracked, and we integrate their recommendations into your ongoing management.
Many medical aids contribute to chronic benefits and routine monitoring. We provide transparent quotes in advance and assist with pre-authorisations where possible. Note that certain labs or scans may be billed separately by providers.
